Availability of Form 1095-C for the 2025 Tax Year

IMPORTANT NOTICE

Copper Canyon Fire & Medical District is providing this notice in accordance with applicable Internal Revenue Service (IRS) regulations regarding Affordable Care Act (ACA) reporting requirements.

Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) for the 2025 calendar year is available upon request.

The District is not automatically mailing Form 1095-C to employees unless a form is requested. Employees and former employees may obtain a copy upon request.

How to Request a Copy

To request a copy of your Form 1095-C, please submit your request by one of the following methods:

Please include the following information in your request:

• Your full name

• Current mailing address

• Phone number

• The tax year requested (2025)

 

A copy of your Form 1095-C will be furnished within 30 days of your request, or by January 31, 2026, whichever is later.

This notice will remain posted in accordance with IRS requirements.
